DigitalHealth.sg today announced AI Chat for Chronic Care, clinical guidance tool designed to help primary care teams triage, monitor, and coach patients living with long-term conditions. The chat interface extends the company’s existing clinical intelligence platform, translating complex recommendations into practical, guided steps that can be actioned during virtual visits or in-person consults.
Clinical guardrails in a conversational workflow
Built on the same health processing units (HPUs) that power DigitalHealth.sg’s chronic disease API, AI Chat for Chronic Care interprets labs, vitals, medication histories, and patient-reported outcomes before proposing next steps. Care teams can present relevant clinical histories, and receive guidance mapped to Singapore, ASEAN, and international guidelines.
“Doctors and nurses crave fast, reliable support when managing complex chronic conditions,” said Dr. Jordan, founder of DigitalHealth.sg. “AI Chat surfaces the ‘why’ behind every recommendation and keeps clinicians in control, ensuring patients receive care that is both personal and protocol-aligned.”
The tool is available inside Remedy AI, DigitalHealth.sg’s white-label telehealth experience, and can also be embedded in partner platforms through a secure SDK.
Pilots underway across the region
DigitalHealth.sg has initiated pilots with integrated care teams in Singapore and Malaysia to validate AI Chat workflows for diabetes, hypertension, and heart failure. Early results indicate a 30% reduction in time spent preparing chronic care plans, freeing clinicians to focus on motivational coaching and symptom review.
Health systems interested in AI Chat can request a guided demo, receive partner onboarding materials, or enrol in DigitalHealth.sg’s chronic care accelerator program.
